Abstract
The triad of thrombocytopenia, microangiopathic hemolytic anemia, and acute nephropathy of the hemolytic uremic syndrome(HUS), represents the final common pathyway, Classic HUS is largely a disease of infants and young children, and is usually
preceded
by a gastroenteritis prodrome. Secondary HUS, however, are usually seen in adults and can be result of, or associated with pregnancy and the postpartum period, certain drugs, including cyclosporine, mitomycin-C and oral contraceptives; malignant
hypertention; collagen vascular disease; malignancies; primary glomerulopathises. There is general agreement that the probable initial pathogenic event is injury to the endothelial cells of the renal microvasculature.
We report a patient who developed HUS after adjuvant chemotherapy which contained mitomycin-c and treat him with plasmapheresis in early phase.
It is show that this type of early phase plasmapheresis have marked effect on renal insufficioncy as weh as the reversal of microangiopathic hmolytic anemia and thrombocytopenia. So, The early intensive plasmapheresis should be considered as a
first
line treatment of HUS.
